Fill out the form completely, and print it on white paper.
- If you are a medical review officer or breath alcohol technician, use the Positive/Refused Drug/Alcohol Test Report.
- If you are an employer or motor carrier, use the Positive/Refused Drug/Alcohol Test Report by Employer.
