Learn how to add your blood type to your driver license or ID card.

You can choose to add or remove your blood type on your driver license or ID card. This designation can support blood donation in our communities and serve as a reminder of your blood type.

Where to add your blood type

You can add your blood type to any license, ID, or permit we offer. This includes:

  • Driver license
  • Enhanced driver license
  • Commercial driver license 
  • ID card
  • Enhanced ID card
  • Learner permit

Add your blood type

You can only add your blood type to your license or ID in person at a driver licensing office.

Visit a driver licensing office and bring one of the following:

  • The Blood Type Designation Request form, filled out and signed by a licensed medical provider
  • A donor card, email, or letter from a blood donation center showing your blood type
  • A digital record (for example, on your phone) from a blood donation center clearly showing your blood type

To remove your blood type, visit a driver licensing office. Licensing fees may apply.
