Ignition Interlock Status Verification
We must receive proof that an ignition interlock device has been installed before we can reinstate a driver license that has been suspended or revoked for a DUI conviction, a refusal to take drug or alcohol tests, or entering a deferred prosecution program.
The Ignition Interlock Status Verification form allows an ignition interlock company representative to:
- certify that a Washington State Patrol certified ignition interlock has been installed on a vehicle.
- OR
- certify that an ignition interlock is no longer installed or functioning.
